The HERO Program

The STFD HERO (Health, Education, Resources, and Outreach) Program is a voluntary, non-emergency initiative available to all residents of The City of Sylvania and Sylvania Township. The program’s goal is to help residents maintain safe and healthy living conditions in their homes.

HERO personnel assist with a variety of needs, including:

· Reducing risks related to frequent falls

· Providing medical education and support for chronic health conditions

· Connecting residents with community resources

· Installing grab bars and other mobility aids

· Advocating to ensure individuals receive the services and equipment they need
